Our Practice

In 1993, Dr. Chavez was honored to be one of the founding faculty members of the Rush-Illinois Masonic Family Practice Residency. During this time, he developed several programs including the Sports Medicine Curricula and the three-year Procedures Curriculum, both of which were very well received. Dr. Chavez went on to author two Office Surgery chapters in Rakel’s Textbook of Family Medicine.  These curricula and publications were important in attracting post graduates to the program and helped open the door to other academic engagements. 

While he continued to enjoy teaching, precepting and mentoring, Dr. Chavez was also eager to move in a different direction.  In 2010, he founded M. Chavez, MD, SC whose sole purpose was to offer complete, authentic, evidence based care with excellent customer service.  The focus would be on preventive health, procedural medicine and in the diagnoses and management of dermatological and musculoskeletal conditions including their rehabilitation. Today, each member of the staff at M. Chavez, MD, SC embodies a similar philosophy of compassionate, quality care.  The clinic is proud to offer patients a variety of services and is committed to every patient’s individual needs.

Dr. Chavez, MD, FAAFP

Over 28 years of experience in Family Medicine, Dr Chavez specializes in family medicine, non operative orthopedics and minor outpatient surgery. His mission is to inspire those seeking their optimal health, wellness, and healing through efficient service, quality consultations, and my core values: integrity and respect.


Vanessa Flores, NP

Vanessas career spans over 6 years, having experience in the progressive care, cardiac and pediatric units including motherbaby. Vanessa has over 3 years of practice experience, having worked at an immediate care center for 2 years as a Nurse Practitioner. During this time, she maintained her academic interests by teaching classes, clinicals, and laboratory at both Resurrection University and Triton College.


Elizabeth Casabel

With 17 years in healthcare, Elizabeth has gained experience in providing compassionate care as a nursing assistant for 2 years and an RN for 17 years having worked in the Neuroscience, Hospice and Postpartum care. She enjoys providing reliable comprehensive care, disease and illness prevention and patient education to diverse populations. Elizabeth believes that excellent continued communication will help create solid working relationships and improved patient care.


Cathleen Bender

Cathleen has additional certifications as a 200-hour Yoga teacher, advanced Craniosacral therapy and Visceral Manipulation from the Upledger Institute and over 160 hours of training in Myofascial Release with John Barnes. Cathleen is passionate about helping patients reach their optimal level of health and happiness by a combination of balancing the fascial restrictions and optimizing the effects of the lymphatic system.


Visenta, Office Manager/Registered Nurse

Visenta was born and raised in Chicago. She has been in the Medical Industry since 2005 where she started as a Medical Assistant helping in activities of daily living for elderly Alzheimer's/dementia patients. After receiving her degree as a Registered Medical Assistant, she worked in various medical clinics assisting physicians and surgeons in the set up and implementation of different medical procedures from knee Injections to Endo-venous Laser Therapies. 

Over the years, her demeanor and compassion coupled with an innate ability to manage and multitask found her move up quickly to the managerial level positions. As Office Manager she takes on the responsibility to coordinate and supervise all office activities and procedures and makes sure support staff gives excellent patient care and acts professionally at all times. 

She's a proud mother of young man that takes pride in his development. In her spare time, she enjoys spending time with close family.  She brings a genuine enthusiasm to all aspects of her work.  Her infectious good nature is a joy to all who work with her.


Ivette, Referrals Coordinator

Originally from Puerto Rico, Ivette settled in Chicago in the early Seventies to raise a family as a stay-at-home mom.  She went back to school and took an accelerated course on becoming a Medical assistant in order to satisfy a passion to help others.

Ivette graduated from Everest College 2010 and started working in the medical community initially for Access Community Health Network and then for On-Site Health in 2012 where she worked one-on-one with a wound care specialist. Duties of this job also included communicating personal wound care information and treatment protocols to nursing agencies, providing visiting physicians a confirmed schedule of patients and computer data management and telephone prescription monitoring.

Ivette became part of Dr Chavez group in 2015. She was initially hired as an MA and has since received her R.M.A and the title of Referrals Coordinator. She works as our liaison for hospitals, specialists and our patients, supporting our clinicians and patients in the web of referrals and prior authorizations.She enjoys her work with us and still finds time to spend with her family


Marylynn, Medical Office Assistant

Marylynn was born and raised in Chicago and has called the Windy City her home ever since. She has always had an interest in caring for family and others, an interest that led her to pursue her studies to the medical field. Prior to joining out staff, Marylynn had worked for a pediatrics clinic followed shortly thereafter with work at a private OB-GYN practice. Her experience in these fields along with her personal demeanor are welcome in our practice. 

Marylynn is our Operations Coordinator. Some of her duties consist of medical inventory, coordinating and scheduling meetings with medical representatives, assisting or finding an assistant for our clinicians with in-office medical procedures, and scheduling and performing X-rays and EKGs for our patients. Marylynn's goal is to continue growing and branching out within the medical field, and ultimately further her medical career and become a registered nurse.

She is a proud mother to 3 young adults, and she spends much of her free time with her children and husband.
